Records may also be submitted in writing to the Point Pleasant First Aid & Emergency Squad c/o Records Department PO Box 1744, Point Pleasant Beach NJ, 08742. Requests must include either a signed HIPAA release form or a subpoena, as well as the patients name, incident date and location.
